How Can I Change My Beneficiary?
Depending on the form of annuity you elected at the time you commenced benefit payments and the provisions of the group annuity contract, you may or may not be eligible to change your beneficiary. For some forms of annuity, there is no need for a beneficiary. If you are married and looking to designate someone other than your spouse as beneficiary, your spouse must sign the 'Spouse Consent' portion of the form. If you have already designated a beneficiary and would like to make a change, click here to download a Beneficiary Designation form. After you have completed the appropriate sections, please mail the form to the address referenced on the form.
Please note, a joint annuitant may not be changed unless the provisions of the group annuity contract specifically state that this can be done. If a change were possible, it would specify so on your Retirement Certificate which was issued to you with your first payment from New York Life.
